Patient presented for hygiene cleaning, periodontal charting, and periodic exam. Has had several crowns/fillings/restorations/extractions. Getting his mouth back on track, doing a great job. Noticed a filling feels loose when flossing, but felt fine during cleaning and exam. Will call if notices it becoming bothersome.

The patient is present for Extraction of J, the tooth was elevated and removed from socket with no complications Ms. Victoria was stable after giving 100% oxygen for 10 minutes and once treatment was complete. After being scheduled for cleaning again, the patient was instructed to call if they encounter any problems after extraction.

Patient presented with extensive decay. Treatment completed was crown/build up and two fillings. Anesthetic was given. Patient tolerated treatment well. Discussed future treatment with mom.

Patient came in for periapical first film and pupal debridement on 28, followed by a sedative filling. Presented treatment plan for root canal and crown substrate on tooth #28.

Patient came to our office today for a bridge preparation appointment. The assistants performed extractions, prepared Pontic crowns, and temporary porcelain crowns to prep the patient's mouth for her bridge delivery in the future.
