The patient came into the office today to get a new white porcelain onlay done. Removed the decay and used the dental composite to build the tooth back up. Took a digital scan of the tooth and built a new white porcelain onlay. Placed porcelain onlay with permanent dental cement. Checked the bite and made adjustments. The patient was happy.

Female, adult patient, presented for new hygiene foundation appointment. Took xrays, periodontal charting, and intraoral camera pictures. Patient having sharp pain when eating on left side, diagnosed onlay restoration. Patient will come back for onlay restoration, cleaning, and comprehensive exam.

The patient came in for a crown and an onlay. We removed decay on tooth and used composite to build tooth back up. We scanned the crown prep on the Cerec and milled it using emax material. We placed crown on with bruxzir permanent cement. We checked bite with blue paper and bite was great.

The patient came into the office today to have an extraction. Numbed patient and used elevator, drill, and forceps to pull the tooth. The patient felt great after. Talked with the patient about dental implants to restore that area once that area is healed. Sent home with post-op instructions.

I performed a foundation appointment for a new patient. I took X-rays and IOP, and Dr. Greer went over the results. He advised that the patient had good teeth and needed deep cleanings in specific areas. He also mentioned the use of a waterpik to keep the teeth clean. I will set up a follow-up appointment for the patient after the deep cleanings.

We completed a new patient foundation appointment, which included taking FMX and IOP. Dr. Greer went over X-rays and IOP, and advised that the teeth looked really great. He measured pockets and advised that 10-11 had a widened PDL, 27-D had slight bone loss, 15-M had radiolucency on X-ray, and 30-O had radiolucency under filling. PTR for foundation appointment number 2. We also added a night guard to the TX plan due to signs of grinding teeth.
