pt present at office for extraction Dr. Greer Extracted #(s) with no complications using__forcep______________. Hemostasis achieved. Bone Graft Placed at site #_19_ To preserve the site and occlusal planes for future restorations. Hemostasis achieved. Post op instructions given to pt with extra sterile gauze. Pt stated they felt stable.

Female, adult patient, presented for hygiene cleaning, periodontal charting, and periodic exam. No pending treatment. Has two broken crowns that she knows about, not bothersome, doesn't want to do anything to them at the moment. Improved hygiene since last visit.

As a dentist, I completed a root canal and a crown procedure to restore a patient's smile. I removed the decay and prepared the tooth for a root canal. I then added a dental post inside the tooth to give it more strength. I used a Cerec machine to scan the tooth and create a new porcelain crown. The patient was happy with the final result.

Both patients were seen today to have their first cleaning and comprhensive exam with the hygienist and dentist. They are both scheduled to come back for there six month cleaning appointments. When ready they do have come filling appointment they can get scheduled as well.

Present today for limited exam #9 missing filling, Dr. Greer clinical exam initial replacement will be a crown preparation and build-up. Replacing with another Mesial, Facial, incisal, lingual filling would not last on #9.

The patient is present for delivery of #9 crown, before cementation we confirmed that Mr. Brian was pleased with aesthetic look and color of #9 crown . Dr. Greer gave his clinical opinion moving forward #9 was cemented Relyx ultimate Cement; patient was satisfied with remake of #9 crown.
