The patient presents a dental implant placement on teeth 28,29, and 30. Measured and prepped bone for correct size dental implant. The procedure was easy and the patient had no complications. The patient will return to have the dental implant restored with a new white porcelain implant crown.

The patient came into the office today for a tooth extraction. I numbed the area around the tooth and used an elevator, drill, and forceps to pull the tooth. The patient felt great after the procedure. I talked to the patient about dental implants to restore the area once the area is healed. I gave the patient post-op instructions to take home.

The patient came in today to discuss dental implants and porcelain crowns. We went over the process of all the steps we take and how we can help restore their teeth and give them an esthetic and functional smile again.

The patient came in today to have a dental porcelain onlay put on. Placed on with permanent cement and checked bite with blue paper. The patient was happy with the fit and look of their dental onlay.

Discussed nitrous with patient for extraction. Periodontal readings decreased after full mouth deep cleaning, still has loose/flappy gingiva.

THE PATIENT WAS SEEN IN THE OFFICE TODAY FOR A CROWN DELIVERY OF A PORCELAIN CROWN THE CROWN WAS MADE IN HOUSE THE CROWN WAS CEMENTED WITH PERMANENT CEMENT THE CONTACTS WERE CHECKED AND THE NECESSARY OCCLUSAL ADJUSTMENTS WERE MADE
